Admission Requirements 

​REGISTRATION FEE – A non-refundable registration fee of  $100 ($150 for a family) is to be submitted with the application form. 
 
SCHOOL RECORDS – Copies of previous school(s) records including all  educational and diagnostic testing, transcripts, and report cards are to be submitted. Not applicable for Pre-Kindergarten/Kindergarten students. 

STUDENT QUESTIONNAIRE – Required for Upper School students (grades 6-12), this form is completed at the time of application. 

BIRTH CERTIFICATE – A copy of the student’s birth certificate is  requested from all students. 

SOCIAL SECURITY CARD– A copy of the student’s social security cards  is requested from all students.

HEALTH RECORDS – Health information forms and immunization records   are required before the first day of class. Forms may be Xeroxed or transferred   from the previous school. Immunization records must demonstrate that the child   is current on the MMR, polio, DTaP/Td, Varicella (chickenpox), and hepatitis B  vaccines. If the child has had the chickenpox illness a varicella vaccine is  not  required, but the immunization record must document the month and year of  the  chickenpox illness. Immunizations records must include month, day, and year  of  each shot and the health care provider’s signature or stamp. Children who  are  not in compliance with state immunization guidelines will not be allowed to  attend school. 

STANDARDIZED TEST RESULTS– The most recent copy of IOWA, LEAP or  other comparable annual standardized test.

ENTRANCE INTERVIEW– Student and parents will meet with the  admissions committee for an informal interview. 

PARENTAL PERSPECTIVE-A parental perspective form is included in your enrollment packet.  This form is used to provide us with information about your child so that we are better able to meet their needs.  This should include areas of strengths and ability, as well as areas of concern.  Please indicate any goals that you have for your child both spiritual and academic in nature.

STUDENT AGREEMENT OF CONDUCT - In order to be admitted into Family Community Christian School, all students and parents of students in grades five through twelve must sign a student agreement of conduct.  This is used to ensure that all students/parents understand the standards that FCCS upholds admitted students to.

PERMISSION FORM - In order to be admitted into FCCS all students must have a school permission form in their permanent file.  These forms will be carried with teachers on all off-campus activities.  This ensures the faculty is able to contact parents and emergency contacts if the need arises.

MEDICAL INFORMATION/PERMISSION FORM - The medical information/permission form is used to give the school office permission to give your child medication such as Tylenol.  This form is used to give the parent the option to decline or allow this service by FCCS.
